    • Annual Forms Overview

Families of returning 7th graders need to complete two forms:

      ☐ Annual School Form

☐ Free and Reduced Priced Lunch Form

    • Free and Reduced Priced Lunch Form

    • The following document is the Federal Free and Reduced Priced Lunch Form. It is used for two primary reasons. First, Kairos uses it to determine students who receive federally-reimbursed breakfast, lunch, and snacks. This is how we ensure that every child, regardless of background, receives nutritious brain food. Second, we use this form to collect the state and federal dollars that Kairos needs to build a school fueled by rock star teachers and top-rate resources.

      Automatic Qualification for Free Lunch

You can qualify by income, or you may be pre-qualified! If any member of a household receives food assistance from the government, all children in that household are eligible for free lunch. Examples of those programs are:

      • Food Stamps
      • Supplemental Nutrition Assistance Program (SNAP)
      • Temporary Assistance for Needy Families (TANF)
      • Food Distribution on Indian Reservations (FDPIR)
To receive free meals, we just need the member's food stamp, TAFI or FDPIR case number.

Furthermore, children automatically qualify for free meals if they meet any of the following conditions:
      • Foster Care

      • Runaway and Homeless Youth

      • Migrant Children

If a child in your household meets those criteria, please let Kairos know.

      Have Questions?

Please fill out this federal form carefully and accurately! If you have any questions, don't hesitate to contact Kairos at 314-252-0602.

Kairos will review the applications and determine eligibility. If you are dissatisfied with the ruling, you have the right to discuss the decision with Kairos’ Hearing Official. Please submit a support request via the Get Help form or contact 314-252-0602.
